Project Summary: Identify at least three organizations in your community that sponsor health promotion and disease prevention programs. For each organization, provide information on the type of program, the type of interventions, eligibility for services, and costs of services. Determine whether the program activities focus on self-care. If possible, interview the director of one of the programs to determine the effectiveness of these activities in your community. Discuss your findings in your paper.
