For some context, your Project, Part 3 (FINAL PIECE- not due yet!) will be a FACT SHEET that could be dispersed to others to inform them about the population you chose, the problem, the statistics, potential solutions (strategies), AND local/government/ online resources they could access. I am including two examples of 3-4 page fact sheets. Please note that a fact sheet consists of: A. Rationale- why is this a problem and why do we care? -identifies the target population (who the intervention is for) -identifies the target venue (where the intervention will take place) - the obesity/diabetes statistics for this target population you selected (cite) B. What can be done? -helpful strategies for preventing/ combatting this chronic disease (cite) -local/government/online programs and resources that can be used (cite) C. References **In addition, a FACT SHEET is visually-appealing, easy to read, easy to understand, and has a very specific point it is trying to get across. For this assignment- Project, Part 2- you will: Focus on Part B (above)- Describe the mission and activities of (a) local or federal organization(s) devoted to reducing or preventing the health disparity in your target population. This MUST be a real program or organization. This can be an online program, or a government program, or a local program. You must be able to describe it and how it would be useful for your population. IMPORTANTLY- you must explain what this is and why you chose it. Make sure to cite ALL references as appropriate. Please check for grammatical and spelling errors before submitting. This submission should be one page in a Word document uploaded. Use 12-point font.
