This agency could be an adult day center, an assisted living facility, a retirement home, a congregate meal center, a nursing home, a personal care home, or some other type of agency that serves the elderly population. Perhaps this is a facility in your community that you drive by daily but never knew what services are provided. You will have a learning experience with Amazing Place Adult Day Center this semester, so DO NOT choose this facility for this assignment. Review online information about the agency/facility you have chosen and answer ALL the following questions. Number the questions and include the question followed by your response. Your answers should be in your own words; do not copy and paste. State the name, address, phone number and website or URL of the agency/facility you have chosen to discuss for this assignment. State the mission or purpose of the agency in your own words. Describe the type of clients and population served by the agency. Discuss the services, programs, and types of activities provided by this agency for their clients/ participants and the community. Describe the type of staff employed by this agency (include nursing and non-nursing such as dietary, housekeeping etc.). If a nurse is employed by this agency, describe the nursing position(s) and roles. Identify the cost of services and how attendance is funded i.e. private pay, Medicare, Medicaid, long term care insurance, or other sources of funding.
