Interventions are detailed and specific for the patient in the case. Identifies potential problems and expected early changes in patient’s condition. Communicates to members of the healthcare team concisely and effectively using SBAR by summarizing relevant clinical data and correctly interpreting current clinical status.
Evaluates performance on case by thoroughly reflecting and identifying strengths and weaknesses in any of the following areas addressed in case: clinically significant data; caring & art of nursing; identifying potential problems; interpreting diagnostic tests; identifying primary patient problem, formulating a nursing plan of care; SBAR; educational/discharge priorities. Demonstrates commitment to ongoing improvement with detailed and specific plans to eliminate weaknesses (identified in reflection) and apply what was learned to care for future patients.
Completes and turns in on time all required clinical activities included in the assignment. Please see the individual assignment description for specificities.
