I have a project of a Example Factsheet, IEP, and Exercise for a person who suffers from a stroke. I had someone do it for me but they didnt include everything I wanted. I will be providing what the homework NEED to have and to examples. These examples is how exactly how I want it. Can someone please fix it.

These are headings Also they did the Individual Exercise Prescription a little wrong The exercise is also formatted wrong I want it exactly formatted like the examples I will be provided with all headings and underlined with plenty of information please.
