In the who/what/where/when section, I'd like to see more detail about the use of mobile clinics and public health transport. You mention they will be involved in the process - tell me how they would be involved, what specifically they would do, who would be involved, where will they go/be to reach the members of the community, etc.

  • I'm struggling to see a direct connection with the proposed data collection plan, and the aim/outcome you are hoping to achieve. If your goal is to increase access to care by 25%, you will want to collect data on the number of individuals in the underserved community who have access to care as a result of your proposed initiatives (community outreach, mobile clinics, medical transport). You would want to first capture the percent of people in the respective area who currently have access to care, and then measure the percent of people in the area who have access to care at periodic intervals (perhaps at six months, 12 months, 18 months, and 24 months) leading up to the two year time frame specified in the aim, to see if your programs are helping to increase access to care.

  • Other data you may want to collect might be the number of educational leaflets distributed, the number of communication interactions that occur through social media, the number of mobile clinics held, and the number of times public transport was utilized to facilitate transportation to necessary care.
