Immediate needing someone to complete a Psychosocial Assessment and a Master treatment Plan. Because the assessment and treatment plan are to be done on the same person you MUST complete both assignments. There is also 3 consent forms that must be signed, scanned, saved to a file for submission so there will be 3 differe t files needed assesstment, treatment plan, and consent forms. I will give the online learning portal information because there are significant resources and reading materials and the proffessors files that can assist with completion and for you to obtain the consent forms. I am also attaching some files for review. The assignments can be done on anyone of your choice (real/imaginary) as long as it is done completely and within the correct guidelines. I will need everything completed and uploaded via HFY by 09/27/14 by 11 pm Pacific Daylight time. No exceptions I need time to review and revise if necessary. However the files uploaded gives examples of a proper and complete Psychosocial Assessment and Master Treatment Plan you have the templates for both you will do them on and dont forget to fill out the consent forms with your patients signiture and you can just sign the case worker's signiture as Mrs. Givens. Also my class is in addiction studies as you may have noticed when logging into the online learning portal so somehow you need to tie some kind of addiction in the assessment and treatment plan ex:alcohol, cigarettes, cocaine, food, etc. Understand?

I explained that it is full of resourses pertaing to the project in the event you felt you needed to explore further. Secondly, I have reviewed the drafts the only suggestions I have are in the area HISTORY OF ABUSE: PHYSICAL / SEXUAL / EMOTIONAL: (if applicable) that section is suppose to be about the patients history only not his girlfriends so if HE has never been sexually, physically, or emotionally abused by anyone (ex: mother, father, uncle, girlfriend, etc.) then you should just state client has no history of abuse however if HE has then this is where you would state for example Client previously served time in a correctional facility where he was sexually asulted by an inmate but has never openly admitted this to his parents for reasons of embarrasment. Client states his relationship with his mother is healthy however the relationship with his father is strained due to the verbal and physical abuseof him by his father when he was an adolescent. Client recalls his father use to strike him and call him mean and ugly names when he became angry at him... This would be a proper way to adress that area for future references:) you may revise as you please. Additionally your MTP states that your client is unruly and expresses that he does not want to seek treatment and when questioned he becomes agitated yet your Psychosocial assessment states he knows he needs treatment and is willing to cooperate. Both areas need to state the same rationale because your assessment is your first contact with the client and your treatment plan is made based upon what you learned in the assessment. There are slight areas of improper grammar but other than that great job!! Also please do not forget to SIGN the clients signiture(Mr. Brown) and the Caseworkers/ casemanager signiture as Mrs. Gvens as well as upload the signed consent and treatment forms the only areas that need to be signed are the clients and casemanager/caseworker DO NOT worry about any other signiture request including the physicians remember this is the caseworkers treatment objectives.
