Primary Prevention Teaching Plan Rubric: Refer to page 415-416 as example for what is expected in the Primary prevention teaching plan from Weber, J. & Kelley, J. (2018). Health Assessment in Nursing (6th ed.). Philadelphia: Wolters Kluwer Health (LWW). Choose a patient or a family member or acquaintance for this assignment. Introduction: Identify disease Discuss morbidity, mortality, significance data for the disease - Identify the pathophysiology of disease briefly Healthy People 2020 Goals: Identify healthy people 2020 goals for the disease Conduct risk assessment for the patient in clinical case study Identify risk factors that the patient has in the case study: - Note that if your patient in the case study does not have any risk factors, they are not a candidate for primary prevention teaching on the chosen topic. Develop at least one teaching material. It can be a handout, brochure, slides, picture and add it to the end of the assignment as an exhibit. Cite at least two current evidence-based websites or journal articles that support content to be taught. Use correct grammar, spelling and APA format.
