  • Each medication should have three lines of information 1. Name (generic/trade) 2. Indication for use 3. Nutritional implications The first medication listed says: Glipizide 30 mg daily (daily) That is not sufficient. Most of the drug information is what I had done myself. It was up to you to input the additional information and add more relevant medications. This patient should also be on nutritional supplements relevant to bariactric surgery.
