The first thing I see is that you did not follow the assignment instruction. You are to use the same program you developed in the logic model in Module 3 and created a budget for in Module 4. Your specific objective is an enormous goal that even the federal government could not address. I encourage you to go back to the module 3 specific objective and logic model. Use the feedback I provided to improve the specific objective for this assignment. When you open the template, you will see that there are columns for measures. Select one per column that is relevant to your project. For example, under Outcomes Short term what knowledge do you want to see changing as a result of your program. From there complete the rest of the information in the chart- how often will you measure, where will you get the data, etc.

You have developed a logic model (Module 3) and a budget (Module 4) for a public health program. Now you will develop a Monitoring and Evaluation (M&E) plan for that same project. In your plan, provide at least one indicator, measurement frequency, data source and collection method and how will the measurement be used for the each Outputs (Activities, Participants) and Outcomes (Short, Mid and Long Term). Challenges/Problems: In creating the framework, you probably have found challenges, e.g., gaps in existing data, you did not budget for M&E activity, attributing outcomes to program activities independent of external forces. Describe these challenges and how they could be addressed.
