Select and describe a community
The community can be a city, state, region, county or country. The group should select a community about which information is accessible using internet resource. Describe the community physically, geographically, and socially (including the age structure, religion, and factors that give the community its unique characteristics and health factors). If you find a community health assessment, you can use it, but you must cite the document and provide addition sources of data that contribute your health assessment.
2.Characterize the population
Describe the community in terms of demographics (e.g., age, sex), education (e.g., literacy rate or high school graduation rate), culture (e.g., religion, ethnicity) and economy (e.g., employment, average household income, industry). Then provide addition factors that might impact health (smoking rate, number of grocery stores, substance abuse, incarceration rates) and relevant health statistics (e.g., life expectancy, infant mortality).
3.Describe the morbidity and mortality of the population.
In this section describe the diseases and infections that effect the community. The mortality rate should be used to describe the age and causes of death in the population.
4.Community Engagement in the CHA Process
Describe how and with whom you will share the information above in a process to garner community input in priority setting and ownership of the CHA implementation plan.
This community health assessment may be used in your final paper, which is not a group project. So you will want to devote extra effort into selecting a community for which health assessment data is available.
