In this assignment, you will design a program plan for addressing the community issue you have researched in Workshops One and Three. This assignment will provide you with an opportunity to apply information from this and previous workshops into program creation. Your instructor will provide written feedback on your program plan that you will discuss and apply during Workshop Five. Upon completion of this assignment, you should be able to: • Explain how the data collected is used to design a social services program. Resources • Textbook: Conducting Needs Assessments: A Multidisciplinary Approach • File: Program Template.docx Instructions 1. In your textbook, Conducting Needs Assessments: A Multidisciplinary Approach, read Chapter 10, “Reporting the Findings.” 2. Now that you have data, have analyzed it, and have completed the needs assessment, you are ready to design a human services program. Using information from previous workshop assignments, you will create a draft design for the program for your local community. 3. Download and use the Program Template.docx file to complete this assignment. Instructions are highlighted yellow in the file. 4. When you’ve completed your assignment, save a copy for yourself and submit a copy to your instructor using the Dropbox by the end of the workshop.
