Health Story Paper & Collage Students explore a personal health topic that is meaningful and relevant. They write a 2 page double spaced paper AFTER conducting an interview and preparing their collage. 1. Decide on a health related topic you want to explore – an injury, illness, trauma, disease, addiction, etc. Feel free to write about something you have been dealing with or are concerned about, a current health issue, something from the past or an aspect of your health you are curious about. Reflect upon the timeline. When did this start? Did it end? Is it ongoing? Is it reoccurring? 2. Interview a family member or close friend who can provide perspective to your story. The person you choose must be someone you feel comfortable talking about the factors that contributed to being sick/hurt and how you got better. This person is someone you feel “safe” with. - Grading Rubric 1- Identify a health topic of your choice, something personal & meaningful to you. Identify the health story and why you choose this topic. Discuss the factors that contributed to being sick/hurt and how you got better. What were the most important factors in the healing process? Feel free to write about a current health concern, something from the past or an aspect of your health you are curious about exploring or adopting a new health practice. 2- Conduct and describe the interview in your paper, provide a quote. 3- Mention the three levels of awareness as applicable to health story. ------------- the health story will be about how smoking affects my body in running and playing soccer, and also I have to exercise more and stay away from fast food
