Case study to include the following:
1. History of the condition, including patient's age, socio-economic status, and at least two co-morbidities.
2. Complete discussion of pathophysiology of any/all disease states assigned.
3. Presentation of all assessment findings that include the patient's history, diagnostic studies, labs, and medications.
4. Nursing assessment, interventions, nursing care prioritized, and outcomes (both negative and positive) based on changes in your patients condition. You must include at least one change in your patient's condition to demonstrate the 'unfolding' aspect of your case study.
5. Patient education and discharge instructions.

This case study should be in a hospital setting with an acutely ill patient that is progressing toward ESRD. Reduced urine output is one of the main problems.
