Instructions:
Students will develop a personalized professional informed consent document. Clinical Mental Health Students and Marriage, Couple, & Family Counseling students should have a paper document while School Counseling Students may use a paper document or a brochure. A template/skeleton example of a paper document is included to get you started. Students should use this template/skeleton example as a guide and should not copy and paste content or sections. Other resources and examples can be found in the text as well as informed consent samples available online.
According to the ACA Code of Ethics (2014), there are specific elements in an informed consent (Standard A.2.b). There are a number of required areas in the Code of Ethics including 1) Purposes, etc. of counseling services, 2) Counselor’s qualifications, 3) Arrangements for services, 4) Implications of diagnosis, etc., 5) Fees and billing, 6) Confidentiality and limits, 7) Client’s rights to information, 8) Client's rights to refuse services, etc. Use these eight areas as the outline of your informed consent. There are other writer’s recommendations that you may choose to include as well.
