Evaluate and manage one patient selected for co-occurring or potential problems in any problem such as Anxiety
Mood changes, especially depression, Insomnia, SMI, Substance abuse, Sleep disorders, Eating disorders, Attention-deficit/hyperactivity disorder (ADHD), Sexual assault,
Psychotic disorders, Grief and/or Intimate partner violence (IPV)
Use the SOAP format( subjective, objective, assessment and plan) to complete the assignment report in a powerpoint presentation. The following should be clearly addressed in your report:
1. Background of the problems supported with strong research and evidence. Highlights disparities in vulnerable
populations and groups. Explains neurobiological basis and genetics/genomics (if relevant) for the problem.
2. S: During your usual encounter with a primary care patient -
a. Obtain usual subjective data (medical, demographic, cultural, social, family, etc) that reveals patient’s
current/co-existing and potential risk factors and vulnerabilities for specific or co-occurring mental
health issues. Issue addressed must include substance use.
b. Demonstrate the use of motivational interviewing in your interaction with the patient. Provide at least
one specific example of your utilization of motivational interviewing skill, type of skill used, its
benefit, how patient responded.
3. O: Conduct examinations and diagnostic evaluation (or recommend) as follows. Address and evaluate relevant current/co-existing and potential mental health risk factors and
vulnerabilities identified. Issues evaluated must include substance use.
b. Objective evaluations must show use of diagnostic tests and clinical practice tools. Provide
neurobiological rationale for test/screening conducted or recommended. Issues evaluated must
include substance use.
4. A: State the diagnosis/es based on data obtained -
a. Specify ICD-10 code for the most likely diagnosis for the patient. Describe specific DSM-5 criteria
for the diagnosis stated.
b. Identify and explain key differential diagnosis/es
5. Plan: Management
a. Provide/Recommend best psycho-pharmacologic, non-pharmacologic, and preventive plan for the
main mental health problem diagnosed. Explain neurobiological basis/rationale for
o ordering diagnostic tests (explain why, if not relevant), prescribing pharmaceutics, management
recommendations.

b. Include plan for substance use or risky behavior.
c. Address management of co-occurring conditions
d. Incorporate efficient use of motivational interviewing communication skills in management
interventions for issues, including substance use or risky behavior. Provide examples showing use of
MI.
e. Clearly demonstrates the use of strong evidence and best practices in your management decision
making. Patient education is addressed in the plan.
6. Organization and Quality of Report:
a. Organized; Has clarity; Fit as a model case
b. Support information with strong references, and sources are properly cited in the APA format both intext and in the reference list.
