Directions for “Life After Wellness” Reflection/Wellness Plan (100 points)
1. Personal Wellness Plan: What happens to your personal wellness goals after you finish this wellness class? First, discuss your current status for each dimension of wellness. Then, establish a short-term goal or long term goal that is realistic and measurable for each wellness dimension.
Example:
Physical Wellness
Current status: I work out once a week but I have made progress on my cardio fitness. My mile walk improved by 30 seconds and my heart rate improved by 15 beats per minute from the beginning of the semester. (HAD data if applicable)
Goal: I would like to maintain a healthy weight of 180-190lbs during college.
Plan: I plan on reaching a healthy weight by exercising 3-5 times per week doing cardiovascular exercises for 45-60 minutes.

2. Health Appraisal Plan: How will you maintain or improve on your health and fitness levels? You will list your results from health appraisal day and set a goal based on these results. If you did the blood draw, list the areas that were outside the normal range and set a goal to help you return to a normal range. If you DID NOT get your blood drawn, you will need to write about some functions of some common nutrients. Use the following categories to answer the questions below: Iron, Vitamin C, Calcium, Cholesterol HDL/LDL. and Antioxidants.

Health Appraisal Day
Flexibility
· Current Status: I wasn't able to touch my toes.
· Goal: My goal is to improve my flexibility and be able to touch my toes with ease.
· Plan: I plan on warming up before exercising and doing static stretching after exercising.
Body Composition or Body Mass Index (BMI)
· Current Status: My BMI was a 22.
· Goal: I plan on decreasing my BMI in the 15-20 range.
· Plan: I plan on exercising 3-5 times a week and limiting my caloric intake to 2000 calories during the week.
Blood Pressure
· Current Status: 120/80
· Goal: I plan on maintaining a healthy blood pressure
· Plan: I plan on exercising 3-5 times a week and limiting my caloric intake to 2000 calories.
Blood Draw Results
· Current Status: My LDL numbers were too high (110 mg/dL).
· Goal: The goal is to eat healthier.
· Plan: I plan on eating healthier by eating green vegetables at lunch and dinner.

List the appropriate information such as, name of the resource or service, location, telephone #, website, hours of operation, etc.
