For your Charlson score formula, there are a number of variables that should not be in the formula or have the incorrect multiplier. The average Charlson score for those who no showed and showed should have been around 8.3 and 4.4, respectively. For example, AU2 (age) is multiplied by 6 when it shouldn't have a multiplier. Moreover, AH:AK were included in the calculation even though they are not a part of the Charlson score formula.

I am also not sure how you calculated the data in Sheet10. I liked your proposed approach for reaching out to patients to get a better sense of why they didn't show. Style of communication) 3/5 - Remember that it is very important to include filters in your database. Otherwise when you sort the data you risk messing up the data.
