1. Clearly identify 8 LOCAL Health and Human Service agencies within New York State. (Located in Manhattan, Queens, Brooklyn, Long Island, Bronx, Westchester or Staten Island) 2. State the purpose of the Agencies 3. Describe how these agencies have impacted our community 4. Summarize each agency’s main purpose 5. Identify the significance that each agency has on the community 6. Describe how each agency relates to different chapters in the Connect Core Concepts in Health (Brief) 16th edition eBook 7. Describe each targeted population for each agency 8. Include all contact information for each health and human service agency, this means the address location, phone number, email, and website. 9. This should be no less than 16 slides. Remember like a presentation you are explaining the agency to me as if I have never heard of it. Examples of Local Agencies: Planned Parenthood, Pride for Youth, Live On NY, Maurer Foundation for Breast Health Education, EPIC Long Island, etc;
