I will focus on the woman’s weight/obesity. With her BMI at 32, she is defined as having obesity. Obesity is currently recognized as a major global epidemic. 70.7% of the American adult population are deemed overweight or obese. Studies have shown that obesity predisposes to hypertension and CVD. A 2-kg (4.4lb) loss in weight would result in a decline of 3.7mmHg in systolic and a 2.7 mmHg in diastolic blood pressure. In addition, structural changes in the kidneys secondary to obesity are significant. Pressure of peri-renal fat, coupled with increased abdominal pressure secondary to central obesity, contributes to altered renal sodium reabsorption. Also, emerging evidence shows that visceral adipose tissue, relative to subcutaneous adipose tissue, is more sensitive to lipolysis and secretes higher amounts of inflammatory cytokines, playing a large role in the CVD spectrum as well. (Wahab et al, 2020). For her height, she should weigh between 104 and 131 lbs (Dept of HHS, 2013). Immediate and strict diet restrictions should be educated to this woman to decrease her weight. Referral to registered dietician would be a suggestion as well. An exercise program should be started as tolerated as well. Increased monitoring of her blood pressure would be addressed as well. Continued and frequent follow-up visits would be scheduled to evaluate.
