Step 1. You will explore the Census.gov website and find demographic details on Newark NJ. For example, identify the sex, age, income, housing, etc. distributions in the area. You may go to any details you would like to include. Discuss disparities in income, race, age, and gender by place (NJ and your selected municipality/town). Compare. Create a power point. Step 2. Next you will explore the county rankings (http://www.countyhealthrankings.org/) and the New Jersey Health data site and identify and discuss the health status, indicators, health problems, births and deaths etc. of the population in your area of choice. Choose one health problem for your action plan. Step 3. Document multiple social determinants of health (both positive and negative) in your community (home or work) with photographs. Use photos you have taken yourself or online. Make sure you explore the municipal website. They may include services you want to include. Screen shots are also acceptable. Approximately 10 photo slides plus additional explanation slides required. Be creative! Additional considerations: Are there signs of diversity and inclusivity? E.g. rainbow flags (LGBTQ inclusivity?) Signs in languages other than English? Step 4. Create an action plan to address your selected health problem and social determinants of health Upstream measures are preferred. Discuss policy and practice implications.
