• This assignment has two steps. First, write an introduction to the agency/organization that is proposing a new program. Include the mission of the agency and any expertise/experience the agency has in working with the issue or the population. Help the reader get to know the agency. Second, create a Problem Statement that convinces the reader that a problematic condition exists for a certain population. Identify and describe the social problem/condition that your program will be addressing. 2-3 pages plus references. Grade will be based on • discussing both the organization and the community based problem – 15 pts • clear organization of material - 5 pts complete a Needs Assessment -The assessment need to include strong documentation, ie statistics and research, of the social problem condition you are addressing. Using a community assessment and research findings, present evidence that shows the normative, perceived, expressed, or relative need. 3-5 pages plus references. Grade is based on: • Selection of appropriate needs assessment tool • Community Need assessment completed with interpretation of findings • Discussion of identified need to address the targeted problem with selection of appropriate grant and or service intervention to address the problem
