I have a concept map that needs to be made up completely. We are at DaVita Dialysis center and all the patients have kidney failure. So you will need to pick either female or male. The patient's BP is always high, and the rest just make it up, please. I have attached a blank template that you have to use and an example to follow.

You need to use words like teach patient, educate patient, encourage patient or advise patient when educating. discharge planning is when you refer patients to different specialists depending on patients needs. for example: cardiologist to monitor the heart or occupational therapist to help with activities of daily living, nephrologist etc. the second nursing diagnosis is wrong please use "Nursing Diagnosis" and when you say as evidenced by, it should be a complication relating to the patients findings and has to be mentioned in the concept map. and so is the interventions for both. I asked you if you were familiar with nursing care plans you said yes? Nursing diagnosis #1: Uremia is a medical diagnosis, you need to use nursing diagnosis.

I did let you know but I didn't get a response and I started working on it myself since my deadline was approaching. It's not your fault. Nursing concept maps are very specific in the way we use the wording and if you are not familiar with it you won't be able to do it.
