Community Health Planning Proposal Due Date is October 10th, 2018 via Hardcopy The scenario is that a donor in the community wants to give $1,000,000 to address a priority health need or issue in the area. As a community health Planner, you have been asked to write a 3-5 page double spaced proposal to make a case for your assigned community to get the funds. You have been assigned a community by way of the Community Health Needs Assessment (CHNA), which basically identifies what the priority health and mental health needs are of the specific geographic area. You are to develop a proposal for getting the funds from this concerned donor. To avoid plagiarism, all content that is incorporated into the analysis needs to be adequately sourced if it is pulled from external sources or is not common knowledge. The paper needs to include the following. For easy readability, answer each question in a separate section within the paper. 1.
