Interview a person with a compromised health state (chronic condition).Discuss the patient’s perception of the disease’s impact on their life from a holistic perspective. Discuss the patient' s perspective of collaboration with the healthcare team (Did the person perceive there was collaboration and did the team include the person as a team member?) Do not include name or identifying information. Briefly discuss similarities and differences between the person interviewed and the person in the memoir you are reading. Reference memoir. Discuss the pathophysiology of the patient’s disease process, anticipated complications, and appropriate prevention strategies (primary, secondary, and/or tertiary). Reference required. Discuss resources in the community which the person accessed. Discuss at least two community resources the person has not accessed which may be beneficial. State how the person would access these resources and whether the person is eligible for services. Share your findings with the person and include their response. Briefly discuss communication strategies used in the interview. APA Style (grammar, punctuation, references, in text citation, logical flow of content). Limited to two pages excluding title and reference pages.
